Out of place downlock pin caused incident with British Airways Boeing 787

A British Airways Boeing 787 was parked at London's Heathrow on June 18 as it prepared for its next flight. The plane was undergoing a procedure to clear messages regarding necessary maintenance. However, maintenance required that the landing gear lever be cycled.
In this type of procedure, a locking pin must be placed on the landing gear, preventing undue recoil. An investigation found that mechanics failed to place the pin in the correct groove in the nose train, which ended up being retracted and caused the accident that culminated in damage to the front section of the fuselage, engines and door. A ladder was attached to the left side of the plane. The accident left two people injured.
In January of last year, an airworthiness directive was issued, with a three-year compliance period, to install an insert over the pin hole, so that incorrect engagement of the downlock pin was avoided so that accidents like this would not occur. .
However, the aircraft in question had not yet undergone the procedure at the time of the accident.
